Who we are

We are a specialist community rehabilitation team. We see people who are or at risk of frailty or falls, people living with long term conditions and people recently discharged from hospital. We work with our patients to achieve their rehabilitation goals, maximise independence in activities and improve quality of life.

Our approach involves working with you to develop a rehabilitation programme and formulate goals. We use a rehabilitation plan to help you set goals and develop a rehabilitation programme. Our interventions may include exercises, education and advice depending on your clinical need. We may use equipment or minor adaptations to help with the rehabilitation programme.

Each team has Physiotherapists, Occupational Therapists, Assistant Practitioners, Therapy Assistants and an Administrator.

What is Community Rehabilitation Physiotherapy?

Community rehab physiotherapy involves physiotherapists providing assessment and tailored rehabilitation to improve mobility, strength, and independence following illness, injury, or surgery. This patient-centred approach addresses personal goals using exercises, equipment, and advice.

What is Community Rehabilitation Occupational Therapy (OT)?

Community rehabilitation Occupational Therapy is provided in the community. OT’s help individuals with injuries, long-term conditions, or illnesses regain independence, improve their physical and mental well-being, and engage in daily activities that are important to them within their own familiar environment

OT’s work with people to empower them through rehabilitation, education and practical support, including equipment provision, to overcome barriers in doing activities that matter to them. This may include washing and dressing, meal preparation and community activities.

Our teams are based in two community sites:

  • North Community team based at Westbourne Green
  • City and South Community team based at Horton Park

We also run services from Westwood Park.

We work Monday to Friday 8am – 4pm.

How to be referred?

For professionals/referrers: referrals can be sent electronically. Referrals can be made via communityrehab@bthft.nhs.uk

Self referrals: You are welcome to refer you or your family member to our service

You must be:

  • entitled to NHS care
  • live in the following postcodes BD1 to BD10, BD12 – BD15, BD17, BD18. We cover some areas of BD16 (Cottingley)
  • have a rehabilitation need and would like to be assessed for our service.

If you would like to self refer please email communityrehabselfreferrals@bthft.nhs.uk

Please include:

  • your full name
  • your address
  • DOB
  • NHS number
  • GP
  • reason for referral

We will then send you a link to an online form for you to complete. We are unable to accept the referral until this form has been completed. The form will have an expiry date on it. Once completed we will confirm by text if the referral has been accepted or if we feel a different team would be better supporting you at this time.

What happens if you are referred to our service?

We may need to contact you to discuss your referral in further detail and check you have been referred to the correct service to support your needs.  We may do this via an online form or telephone call. If we do not need any further information we will send a text to let you know we have received your referral.

Once we are sure we are the right team to support you, you will then be placed on a waiting list. How quickly you are seen depends on clinical need and we will contact you once we have an appointment available. Please note we may arrange an appointment via phone or send an appointment via text or letter.

We may see you at one of our clinic bases or at home depending on your clinical needs. We do sometimes use virtual appointments to support our rehab.
We also run some of our interventions in a group format. Your therapist will discuss the most appropriate place with you.

What happens at your initial appointment

What happens at your appointment will depend on your clinical need.

We will complete a full assessment and will take a full history including your past medical history and health conditions.

We may ask you to complete some tasks such as how you manage getting in and out of bed or on and off the toilet.

We may ask you to complete some further tasks or assessments depending on your needs.

We will then work with you to develop a rehab plan including things we can support you with and things you can work on independently.

What happens after my appointment

Depending on your clinical need you may be seen for ongoing support. This maybe carried out by one of our therapy assistants under the supervision of a therapist.

We may sign post you to other services but would discuss this with you.

Walking aid only

If you have been referred for a walking aid only you may be seen by one of our assistants. They will assess for a suitable walking aid.

Hospital discharges

If you were assessed by a therapist in the hospital you may be seen for follow up by one of our assistants.

They will complete an initial review of your needs and will support you to work towards your rehabilitation goals.

Move to improve group

You may be referred to our group only. You will be seen for an initial appointment and then once established the group is suitable you will be placed on a waiting list. The group sessions run over a number of weeks and it is important you attend as many as possible.

Other support/services

We work alongside colleagues in other services to support your rehab including the falls team, district nurses, social services and GPs.

Please note there is a separate team for patients with MSK conditions who are able to attend outpatients appointments: Musculoskeletal (MSK) Physiotherapy Outpatient Service.

There is also a specialist Neurological team who support people with neurological conditions: Neurological Rehabilitation Team.

Services we DO NOT provide

We are not a rapid response service. If you need support urgently please contact your GP or seek medical assistance.

We are unable to provide major adaptations or care packages. If you require this service please contact Independence advice hub at Bradford Council: Adult social care.

We also are unable to provide wheelchairs. If you wish to have an assessment for a wheelchair visit: Wheelchair Services.
