What should be done before I go home?
If you need to see a doctor for another appointment an out patient appointment will be made for you.
Appointment
this is when you are given a date and time to come and see a doctor again.
Out patient appointment
This means the place you go to see a doctor again
If you have to go back to hospital and need to be picked up at home, ask the nurse if the right transport has been booked for you. This can only happen if the doctor has told you that you need transport.
If you have to go back to hospital and no longer need to be picked up, please call 0845 120 3189 to cancel the transport that was booked.
If you have been taking medication during your stay in hospital the doctor may say you need to carry on taking it.
make sure that you have been given enough medication to take home with you.
When you have no more medication please see your GP.
When you leave hospital you will be given a going home letter.
This will tell you what happened during your stay in hospital.
It will also tell you if anything else is going to happen.
This will help any person who will be involved in your care when you leave hospital.
