Leaving the hospital

What should be done before I return home?

If you need to attend a further appointment for treatment an out patient appointment will be made for you.

If patient transport has been booked (this can only be arrange for patients with a clinical need) for your next appointment, please check with the Nurse/Clerk before you leave the ward/department that appropriate transport has been booked. If transport has been booked and you no longer require it please ring 0845 1203189 to cancel it. If your appointment is for more than three months ahead and you still meet the criteria for transport please arrange transport with the Nurse or Clerk in the relevant area.

The doctor may decide that it is necessary for you to continue taking medicine prescribed to you during your stay in hospital. If this is the case, please ensure you have been given your medication supply before you leave. For further medication, you should consult your GP.

You will be given a discharge summary letter detailing your stay in hospital and subsequent arrangements which will provide information for any healthcare professional involved in your care when you leave hospital.
