General Information

We support the whole system approach to patient care and partnership working by supplying qualified and support staff to secondary, intermediate, primary and social care and education.

We carry out functional assessments with patients to support safe, timely hospital discharge –  washing and dressing, transfers, kitchen, environmental and home assessment visits.

We offer an evidence-based rheumatology and musculoskeletal service which includes assessment, patient education, the provision of small aids to support joint protection, provision of pre-made orthotics and fabrication of custom-made thermoplastic splints.

We carry out cognitive assessment and work with patients with deficits to improve their functional abilities.

We undertake pre-admission screening with patients requiring a total hip replacement patients assessing the patients within their homes, supplying the necessary equipment and adaptations, linking in to support services (both health and social services), to help patients to retain home saftey and in a timely way.
